Plymouth Vikings Football & Cheer – Medical/Liability Waiver

I understand that football and cheerleading are contact sports in which serious injury may occur and give my child permission to participate in Plymouth Vikings Football & Cheer activities. To the extent permitted by Massachusetts law, I release the organization, its Board of Directors, coaches, staff, and volunteers from liability arising from participation, including ordinary negligence, but excluding gross negligence or reckless or intentional misconduct. I authorize program representatives to seek necessary emergency medical care for my child and accept responsibility for medical expenses for which I am legally obligated.
